Hospital Partnership Guidelines
We are excited to partner with you! A donor has been secured for funding in order to serve the families at your hospital. As we have expanded across the U.S. we have determined these are the best ways for us to have an effective partnership. Please read our guidelines below and let us know what questions you have before we mail packets your way. Thank you so much!
-Regular communication to ensure plate orders are generally in line with estimated totals given to Mamie’s and your hospital’s donor when partnership began.
-Based on the number of losses provided during on-boarding, Mamie’s Poppy Plates expects plate orders to start within one month of packets being delivered to your hospital/hospice agency.
-Providing training materials/videos to all nurses and staff who serve in bereavement prior to receiving packets.
-With staff turnover providing ongoing training to any new nurses/staff members.
-Responding promptly to communication from Mamie’s Poppy Plates.
-Hanging posters provided by Mamie’s in a prominent place for nurses.
-Reporting accurate numbers to Mamie’s Poppy Plates’ as needed.
-Providing a secondary contact to Mamie’s Poppy Plates (name/phone/email).
-Providing new contact information to Mamie’s Poppy Plates should staff turn over.
Mamie’s Poppy Plates thanks you for your willingness to partner with us and reserves the right to discontinue our partnership with a hospital that does not adhere to our standard of communication and guidelines. As you have questions please contact us at email@example.com.
